Uses

Myelodysplastic Syndromes (MDS):

Decitabine is primarily used to treat myelodysplastic syndromes, a group of disorders caused by poorly formed or dysfunctional blood cells. It helps by promoting the production of normal blood cells and reducing the risk of the disease progressing to acute myeloid leukemia (AML).

Acute Myeloid Leukemia (AML):

Decitabine is also used in the treatment of acute myeloid leukemia, particularly in older adults or those who are not candidates for intensive chemotherapy. It helps by inducing differentiation and apoptosis (programmed cell death) in cancerous cells.

Chronic Myelomonocytic Leukemia (CMML):

Decitabine is sometimes used off-label for treating chronic myelomonocytic leukemia, a type of cancer that affects blood-forming cells in the bone marrow.

Mechanism of Action

Decitabine works by inhibiting DNA methyltransferase, an enzyme responsible for adding methyl groups to DNA. By inhibiting this enzyme, Decitabine causes hypomethylation of DNA, leading to the reactivation of tumor suppressor genes and the induction of cellular differentiation and apoptosis in cancer cells.

Precautions

Blood Counts: Regular monitoring of blood counts is essential, as Decitabine can cause myelosuppression, leading to low levels of red blood cells, white blood cells, and platelets.

Infections: Patients are at increased risk of infections due to lowered white blood cell counts, so precautions should be taken to avoid exposure to infectious agents.

Pregnancy and Nursing: Decitabine can cause harm to a fetus, so it should not be used during pregnancy. Women of childbearing potential should use effective contraception during treatment. It is also not recommended for use while breastfeeding.

Side Effects

Common side effects include nausea, vomiting, fatigue, fever, and low blood cell counts (anemia, neutropenia, thrombocytopenia).

Injection site reactions, such as redness or pain, may occur.

Less common but serious side effects can include severe infections, bleeding, and liver enzyme abnormalities.
