Exemestane

Description

Exemestane is an oral steroidal aromatase inhibitor that is used in ER-positive breast cancer in addition to surgery and/or radiation in post-menopausal women.

The main source of estrogen is the ovaries in premenopausal women, while in post-menopausal women most of the body's estrogen is produced via the conversion of androgens into estrogen by the aromatase enzyme in the peripheral tissues (i.e. adipose tissue like that of the breast) and a number of sites in the brain. Estrogen is produced locally via the actions of the aromatase enzyme in these peripheral tissues where it acts locally. Any circulating estrogen in post-menopausal women as well as men is the result of estrogen escaping local metabolism and entering the circulatory system.

Applications

Many breast cancers are stimulated to grow by the female sex hormones oestrogen and progesterone. These breast cancers are called hormone sensitive or hormone receptor positive and can be treated with drugs that block the effects of these hormones.

In women who have had their menopause, oestrogen is mainly produced by changing androgens (sex hormones produced by the adrenal glands) into oestrogens. This process is called aromatisation and happens mainly in the fatty tissues, muscle and the skin. It needs a particular enzyme called aromatase.

Exemestane blocks the process of aromatisation. So it lowers the amount of oestrogen in the body. In early breast cancer, taking exemestane can help to stop breast cancer coming back. In advanced breast cancer the cancer cells may grow more slowly or stop growing completely.

Dosages

This is dependent on the individual and, also, the amount of aromatizing compounds the person is running. Generally, a good starting dose is 12.5mg ED (every day) or 25mg EOD (every other day). The user must utilize blood work to see how their body is reacting at the starting dosage they choose and then adjust as needed. Experienced anabolic steroid users will usually find a dose they like and stick to it with no problems. As I said already, if you start noticing joint issues then the dose should be lowered; on the other hand, if you start noticing any estrogen related sides, such as puffy or sensitive breasts or water retention, it's time to increase the dosage.
