Fluconazole [86386-73-4], α-(2,4-difluorophenyl)-α-(1H-1,2,4-triazol-1-ylmethyl)-1H1,2,4-triazole-1-ethanol, C13H12F2N6O, Mr 306.27, mp 138 – 140 ◦C, crystals from ethyl acetate/hexane. The triazole antimycotic fluconazole was first synthesized at Pfizer Pharmaceuticals.

Fluconazole

Antimycotic Properties

Fluconazole is a broad spectrum triazole antimycotic with good in vitro and in vivo activity against pathogenic Candida species (apart from Candida krusei, which is a primary resistant Candida species), Cryptococcus neoformans, and dermatophytes. Aspergillus species mostly show primary resistance towards fluconazole. Secondary resistance development can be observed after prolonged or repeated therapy of Candidaassociated mycoses, especially in AIDS patients.

Indications

Vaginal candidiasisand systemic and mucocutaneous candidiasis. In Cryptococcus neoformans meningitis the substance is mostly used in combination with amphotericin B and flucytosine. Prophylaxis and preemptive therapy in AIDS patients, patients with leukemia, and patients with bone marrow transplants. 

Side Effects 

Fluconazole is well tolerated. 

CNS. Vertigo: 3.7 %, headache: 1.9 %. 

GIT. Stomachache: 1.7 %, vomiting: 1.7 %, diarrhoea: 1.5 %. 

Liver.Rise in transaminases (> 8 % of normal level): 1 %. 

Skin. Exanthema: 1.8 %. 

A Stevens – Johnson syndrome was observed after fluconazole therapy in an AIDS patient.
